In accordance with the Data Privacy Act of 2012 (Republic Act No. 10173), its Implementing Rules and Regulations (IRR), and all other relevant Philippine laws governing the protection of personal and sensitive personal data, I hereby voluntarily provide my consent to AZPIRED INC., AZPIRED HOLDINGS INC., JASA CONSTRUCTION CEBU INC., AZPIRED TRAVELS INC., JASA IT ENTERPRISES OPC, CEBU MEDITEC DIAGNOSTICS CORPORATION, and LA MIRA CORP. (collectively, “the Companies”) regarding the collection, processing, storage, and use of my personal and sensitive personal information.
I understand, acknowledge, and agree to the following:
1. Collection and Processing of Personal Information
The personal information I provide, including but not limited to my full name, address, contact details, date of birth, government-issued identification numbers, employment or educational history, financial information, health records, and other relevant data, may be collected, recorded, stored, and processed by the Companies for legitimate purposes such as employment, engagement, registration, service provision, communication, verification, evaluation, record-keeping, or compliance with legal obligations.
2. Collection and Processing of Sensitive Personal Information
My sensitive personal information, including but not limited to health information, biometric data, financial data, or other private information, may be collected and processed only with my explicit consent, in accordance with the Data Privacy Act of 2012.
3. Purpose Limitation and Legal Compliance
The Companies shall only collect and process personal and sensitive personal information for legitimate and necessary purposes directly related to their services, operations, and compliance with applicable Philippine laws, including but not limited to labor, taxation, health, and safety regulations.
4. Sharing of Personal Information
My personal and sensitive personal information may be shared with authorized third parties, such as government agencies, business partners, service providers, or other entities, strictly to fulfill legal obligations, provide services, or perform legitimate business operations, and always in compliance with Philippine laws on data protection.
5. Security of Information
The Companies shall implement reasonable and appropriate organizational, administrative, and technical safeguards to protect my personal and sensitive personal information against unauthorized access, alteration, disclosure, misuse, or destruction, in accordance with Philippine laws.
6. Rights of the Data Subject
I understand that under the Data Privacy Act of 2012, I have the right to:
->Access and obtain a copy of my personal information held by the Companies;
->Correct or update any inaccurate, incomplete, or outdated personal information;
->Withdraw my consent at any time, subject to legal and contractual obligations;
->File a complaint with the Companies or the National Privacy Commission (NPC) if I believe my personal data rights have been violated.
7. Retention and Disposal
My personal and sensitive personal information will be retained only as long as necessary to fulfill the purposes for which it was collected, comply with applicable laws, or for legitimate business purposes. Once no longer needed, the information will be securely disposed of, anonymized, or deleted.
8. Voluntary Consent
I provide this consent voluntarily and understand that it is necessary for the Companies to collect, process, and use my personal and sensitive personal information to provide services, comply with legal obligations, and perform business operations.
I acknowledge that I have read, understood, and voluntarily consent to the collection, processing, storage, and use of my personal and sensitive personal information by the Companies, in full compliance with the Data Privacy Act of 2012 and other applicable Philippine laws.
By partaking this recruitment process, you agreed to be bound by the principle of non-disclosure and confidentiality. The company expects its employees and partners, including you as an applicant to adhere to the basic rules of confidentiality. This is to ensure that no information of any kind may be disclosed to anyone in any manner without the written consent of Azpired.
Failure to adhere to these guidelines shall be dealt with administrative and legal action to protect any information that may be divulged.
